Shared Residency Affidavit: Ensuring Enrollment for All Students

The Paramount Unified School District (PUSD) understands that not all families have utility bills or leases in their name, even when they reside within our district boundaries. If you share a home with another individual or family and do not have bills under your name, a Shared Residency Affidavit is the necessary document to establish residency for student enrollment.

What is a Shared Residency Affidavit?

A Shared Residency Affidavit is a legal document that allows a student to be enrolled in PUSD when their parent/guardian is living with another individual and does not have primary proof of residency (like a utility bill or lease agreement) in their own name.

Key Requirements for the Affidavit:

To complete the form, please note the following:

Completion by Primary Resident: The individual with whom you share residency (the person who does have bills in their name at the shared address) must complete the bottom portion of the form. This section verifies that you and your student(s) reside at the stated address.

Proof of Address: The primary resident must provide two (2) proofs of address in their name. These documents must clearly show their name and the shared residency address. Acceptable proofs of address include, but are not limited to:

  • Utility Bill: Utility Notice of Service (valid 30 days), Electric, Water/Trash Service, Gas, Landline Telephone, Cable
  • Property tax payment receipts
  • Rental property contract, lease, or payment receipts
  • Pay stubs
  • Voter registration
  • Correspondence from a government agency

Available Choices for Shared Residency:

When completing the Shared Residency form, you will typically indicate your relationship to the primary resident. Common choices include:

  • Living with a relative: (e.g., grandparent, aunt/uncle, sibling)
  • Living with a friend
  • Living with another family
